SSI/SSDI Outreach, Access, and Recovery (SOAR) is a model designed to increase access to disability income benefits for individuals who are experiencing or at risk of homelessness. This guide explains how the model works and how to get involved.

SSI/SSDI

The Social Security Administration (SSA) administers two programs that provide income benefits to those who suffer from disabling conditions that impede their functioning and impact their ability to work.

Barriers to Access

Many people who are experiencing or at risk of homelessness also have disabling mental illnesses, co-occurring substance use, trauma, and/or other medical issues. Accessing the income and health care benefits that SSI/SSDI provides can be a critical first step on the road to recovery. However, their circumstances can impede access to these benefits.

STATE LEAD / LOCAL SOAR LEAD

You coordinate SOAR in your state or community. As the central point of contact, you connect caseworkers, agencies, and new providers, and you keep the local SOAR effort moving. Your organization’s staff process SSI/SSDI applications with the Social Security Administration using the SOAR model.

APPLICATION PROVIDER

You or your organization take intakes and process SSI/SSDI applications using the SOAR model. Caseworkers and community members would be directed to you to begin an application.

REFERRAL CONTACT

You or your organization do not process applications directly, but you can point people to the appropriate SOAR provider or SSA resource in your local area.

TRAINER / RESOURCE CONTRIBUTOR

You would like to contribute materials to the national website — trainings, infographics, documents, PowerPoints, PDFs, or SSA updates — for our Library & SOAR Online Tools.

To be listed, tell us your name, state, organization, and the role (or roles) that fit you. We never publish anyone’s information without their explicit permission. See how listings look on the State Directory.
