About SOAR
SSI/SSDI Outreach, Access, and Recovery (SOAR) is designed to increase access to Social Security disability income benefits for eligible adults who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder.
SOAR has been implemented in all 50 states, the District of Columbia, and Puerto Rico. SOAR-trained case managers have an application approval rate of approximately 65–70%, compared to the national average of 28% for initial applications.

STATE LEAD / LOCAL SOAR LEAD
You coordinate SOAR in your state or community. As the central point of contact, you connect caseworkers, agencies, and new providers, and you keep the local SOAR effort moving. Your organization’s staff process SSI/SSDI applications with the Social Security Administration using the SOAR model.
APPLICATION PROVIDER
You or your organization take intakes and process SSI/SSDI applications using the SOAR model. Caseworkers and community members would be directed to you to begin an application.
REFERRAL CONTACT
You or your organization do not process applications directly, but you can point people to the appropriate SOAR provider or SSA resource in your local area.
TRAINER / RESOURCE CONTRIBUTOR
You would like to contribute materials to the national website — trainings, infographics, documents, PowerPoints, PDFs, or SSA updates — for our Library & SOAR Online Tools.
