12-Step SOAR Workflow

Step 1 Receive Referral

What: Evaluate whether the referred individual is potentially eligible for SSI/SSDI before committing to the full application process.

  • Request that external providers use a SOAR Referral Tool when making referrals, and track all referrals received in a tracking worksheet.
  • Review the SSA's Listing of Impairments alongside any medical records shared by the referring source to assess potential eligibility.
  • If the applicant may have prior or pending applications, submit the SSA-3288 (Consent for Release of Information) to SSA — must be mailed or hand-delivered unless a fax/secure email agreement exists. Alternatively, call SSA at 1-800-772-1213 with the applicant present.
  • If you need more medical information before deciding, request treatment records now (see Step 3).
  • If it's an appropriate referral, schedule an intake meeting and proceed to Step 2. If you cannot assist, provide information on alternative community resources.

Step 2 Complete SOAR Intake

What: Conduct the initial meeting with the applicant to gather background information, establish the working relationship, and obtain all required signatures before records requests go out.

  • Use the SOAR Initial Meeting Worksheet to guide the meeting and take structured notes.
  • Have the applicant sign your agency releases for all collateral sources (family members, employers, treatment providers, etc.).
  • Have the applicant sign two releases per treatment provider: (1) your agency release and (2) the SSA-827. Signing both creates a HIPAA-compliant "compound authorization" that allows you to gather records and re-release them to SSA.
  • Complete and have the applicant sign the SSA-1696 (Appointment of Representative). Hold this form — it is submitted with the complete packet at Step 9, not at intake.
  • Before ending the intake meeting, schedule all weekly follow-up meetings with the applicant through the application period.

Step 3 Request Records from Treatment Sources

What: Gather the medical documentation that will support the disability claim. Request records going back at least two years before the onset of disability.

  • Request records from two years prior to onset (the date the applicant last earned Substantial Gainful Activity — SGA), plus any older records documenting the history of symptoms.
  • Send a records request packet to each treatment source, including: a Cover Letter, your agency release, the SSA-827, and any provider-specific release form.
  • Record every request in the Medical Records Tracking Spreadsheet and follow up after one week for any records not yet received.
  • Review all records received and confirm documented symptoms align with SSA's Listing of Impairments.
Decision point after records review: If symptoms are documented → proceed to Step 4. If symptoms are observable but not documented → continue to Step 4 and recommend a consultative exam in the MSR. If symptoms are neither documented nor observable → encourage the applicant to seek treatment and revisit the application once supporting records exist.

Step 4 Establish Protective Filing Date (PFD)

What & Why: The Protective Filing Date (PFD) is the earliest date SSA can use to determine when benefits begin. Establishing it as early as possible — even before the full application is ready — protects the applicant's potential back-pay period.

  • Go to ssa.gov → "Apply for Benefits" → select "An Adult (age 18 and over)" → "Disability" → "Supplemental Security Income for Disability" → "Apply Online."
  • Agree to Terms of Service → "Next" → "Start a New Application."
  • If the applicant is with you: choose "I am applying for myself" — the applicant signs in to or creates their my Social Security account. If the applicant is not with you: choose "I am helping someone who is not with me" — SSA will follow up with the applicant after submission.
  • After entering basic identifying information, SSA assigns a re-entry number. Write it down AND print the page. You cannot get back into the application without it.
  • Choose "Save & Exit" immediately. Do not proceed further until Step 8 when the full application is ready.
  • You now have 60 days to complete and submit the full application.
Important: You may receive a "Protective Filing Closeout" letter from SSA — this is a standard notice that the full application has not yet been received. You can continue preparing the application normally.

Step 5 Continue Gathering Supporting Documentation

What: Continue building the evidentiary case through regular meetings with the applicant. Steps 5 and 6 run in parallel — best practice is to complete one form per weekly meeting to keep momentum without overwhelming the applicant.

  • Meet with the applicant weekly, using the MSR Interview Guide to structure each session and gather information for the Medical Summary Report.
  • Record notes and information directly in the appropriate sections of the MSR Template as you go.

Step 6 Complete SSI/SSDI Application Forms

What: Complete all required SSA forms using information gathered during weekly meetings. Sign all forms in blue ink to distinguish them as originals, and keep a copy of each signed form in the applicant's file.

  • SSA-8000 (Application for SSI): the applicant signs the paper form. This is submitted with the packet at Step 9.
  • SSA-16 (Application for SSDI): use the paper form as a worksheet only — transfer the information to the online application at Step 8. Do not mail this form.

Step 7 Complete the Medical Summary Report

What: Finalize the Medical Summary Report — the single most important document you write. A co-signed MSR strengthens the claim from "collateral information" to formal medical evidence.

  • Finish writing the MSR and send it to a mentor, supervisor, or your SOAR Center Liaison for review before finalizing.
  • After internal review, send the final MSR to the applicant's treating provider for review and co-signature.
  • Plan for at least a one-week turnaround for the provider's co-signature — build this into your timeline before the 60-day deadline.
Why the co-signature matters: A co-signature by an Acceptable Medical Source (AMS) — such as a licensed physician, psychologist, or APRN — elevates the MSR from "collateral information" to medical evidence. This significantly increases the weight SSA gives to your report and improves approval rates. The MSR and medical records are submitted to DDS (not SSA) once a disability examiner is assigned.

Step 8 Submit the Online Disability Benefits Application

What: Return to the saved application started in Step 4 and complete the full online submission. The applicant must be physically present to electronically sign.

  • Return to the SSA online portal → choose "Return to Saved Application" (do NOT start a new application). Enter the applicant's Social Security Number and the re-entry number from Step 4.
  • Use the paper SSA-16 to complete Part 1: Background Information, and the paper SSA-3368 to complete Part 2: Disability Information.
  • In the Remarks section, enter all three of the following:
    • "SOAR Application – Packet to Follow on or before [Date]"
    • "Represented by [Your Name]"
    • "Applicant is experiencing homelessness" (if they meet SSA's definition)
  • Part 3: The applicant must be with you to electronically sign the Medical Release (SSA-827). This step cannot be completed remotely.
Deadline: The complete online application must be submitted within 60 days of establishing the Protective Filing Date in Step 4.

Step 10 SSA and DDS Process the Claim

What: Once both the online and paper applications are submitted, the disability determination process begins. Your role shifts to active monitoring and follow-up — don't wait for the agencies to come to you.

  • SSA assigns a claims specialist to determine non-medical eligibility. If the applicant qualifies, SSA transfers the case to DDS.
  • DDS assigns a disability examiner to determine medical eligibility. DDS reviews all medical records and the MSR. If needed, DDS can request additional records or schedule a Consultative Exam (CE).
  • Obtain contact information for both the SSA claims specialist and the DDS disability examiner as soon as they are assigned — and stay in regular contact with both.
  • Let them know you are available to answer questions or provide additional documentation at any time.
  • If more than 60 days have passed since medical records were submitted to DDS, follow up with the examiner to ask whether additional information is needed.
  • If a Consultative Exam (CE) is scheduled, both you and the applicant will be notified by mail. Contact DDS to confirm the applicant will attend — a missed CE can result in denial.
Local SOAR Process: In many states, SOAR Leaders have worked with SSA and DDS to establish special handling procedures — including dedicated claims specialists, a "SOAR" homeless flag in Remarks, secure email partnerships, and designated contacts. Step 11 A Decision is Made

What: SSA sends a decision letter to both the case worker and the applicant. Know the possible outcomes so you can respond quickly and correctly.

  • Approved: Benefits go into pay status and are directly deposited into the applicant's bank account. If additional information is needed — including if the applicant does not have a bank account — SSA schedules a Preeffectuation Review Contact (PERC) appointment to finalize payment setup.
  • Representative Payee Required: If SSA determines the applicant needs help managing their benefits, both the applicant and their designated payee must attend an in-person appointment to establish payee arrangements.
  • SSDI Denial (SSI still pending): If the applicant does not have sufficient work history to be insured under SSDI, they will receive an SSDI denial letter while the SSI claim is still being processed by DDS. This is normal — it is not a full denial of both programs.
  • Denied: SSA sends a letter explaining the unfavorable decision. You have 60 days plus 5 days for mail delivery to file an appeal. Read the denial letter carefully — the reason for denial guides the appeal strategy.

Step 12 Track Outcomes in OAT

What & Why: Record the outcome of every SOAR-assisted application. Outcome tracking is how the SOAR program demonstrates its impact, secures continued funding, and improves training and practice nationwide. SOAR-trained case managers achieve a 65–70% approval rate versus the national average of 28% — that difference is only visible through consistent tracking.

  • Record each application's outcome: application date, outcome (approved/denied/pending), benefit type (SSI/SSDI/both), and time from application to decision.
  • If the case was denied and appealed, track the appeal outcome separately.
  • Use your tracking data to identify patterns, improve your MSR writing, and report outcomes to your program leadership and funders.

MSR — Four Mental Functioning Areas

The Medical Summary Report (MSR) documents the applicant's limitations across these four areas, which correspond to SSA's Paragraph B criteria for mental disorder listings. Each description must clearly link the applicant's diagnosed symptoms to their functional limitations. Use direct quotes from the applicant, treatment providers, employers, and collateral sources.

Key rule: An effective description names the diagnosis, shows how its specific symptoms impair functioning, and backs it with observable evidence or direct quotes. Vague adjectives alone ("gets confused," "doesn't like change") are not enough.
Understand, Remember, or Apply Information

Can the applicant learn, recall, and use information — follow instructions, learn new tasks, apply knowledge to tasks?

Ineffective Description

Peter works a few hours a week and has some support at work and home. He likes his job. He can get confused sometimes, but is better when people explain things to him.

Why this is ineffective:
  • Lacks detail about the type and amount of support Peter receives
  • Unclear what types of things confuse Peter or whether it is related to a mental disorder
Effective Description

Peter requires extensive supports at home and work in order to complete multi-step tasks, and frequently appears confused and asks this writer to re-phrase questions during interviews. Peter has a job coach at Home Store, where he works two hours per day, two days a week. Peter states, "I like this job. I get confused sometimes, but they help me a lot. Before I had my coach I messed up a lot." Peter also requires significant supports at home from a home health aide, who cleans his apartment, shops for food, and cooks meals. Records from Health Aide, Inc. document, "[Peter] needs extensive support managing multi-step tasks in his daily living."

Why this works:
  • Numerous details about the support Peter receives to sustain work
  • Detailed examples of home supports and references medical records
  • Clear that Peter needs extensive supports to function in this area
Interact with Others

Can the applicant interact appropriately with supervisors, co-workers, and the public — without conflict, inappropriate behavior, or withdrawal?

Ineffective Description

Ms. Waves has a diagnosis of bipolar disorder. She speaks very quickly, loses her temper, and gets into shouting matches with people. Sometimes, she stays by herself and does nothing. Often she acts differently on different occasions. People around her don't know how she'll be from one day to another.

Why this is ineffective:
  • Does not clearly illustrate the link between illness and impairment
  • A reader might suspect a connection to bipolar disorder, but it is not explicit
Effective Description

Ms. Waves finds that her mood swings are so unpredictable and severe that she never can predict how she will feel. When she is manic, no one can follow her conversation, as her speech and thoughts race so quickly. She quickly becomes irritable with anyone she interacts with and often winds up losing her temper and getting into shouting matches with other people. When she is depressed, all she wants is to be alone and do nothing. She finds that her mood swings can happen within a day's time, leaving her feeling terribly confused and out-of-control.

Why this works:
  • Mood swings clearly described and explicitly linked to her bipolar disorder
  • Shows how mania AND depression each affect her ability to interact
  • Describes interactions with others — key to assessing ability to work
Concentrate, Persist, or Maintain Pace

Can the applicant sustain attention and complete tasks at an acceptable speed — without extra supervision, excessive breaks, or frequent errors?

Ineffective Description

Mr. Washington lost his job as a mechanic. He would get angry, confused, and depressed. He often accused his colleagues of talking behind his back and said that this made it difficult for him to get his work done.

Why this is ineffective:
  • Unclear how anger, confusion, or depression relate to any mental illness
  • Unclear whether cognitive difficulties — not just interpersonal conflict — interfered with performance
Effective Description

Mr. Washington did well in high school and trade school. After his internship at Max's Auto Shop, he was hired as a mechanic and worked there for four years. He initially received a great deal of praise from his supervisor. At age 24, his symptoms of schizophrenia began to emerge. Mr. Washington became increasingly paranoid and depressed and began to hear voices. "I couldn't concentrate because people were whispering about me." "I'm always afraid and sometimes I'm terrified. The whole crew tried to make me look bad so they could give my job to some friend of theirs." As a result of his symptoms, it became difficult for him to complete tasks at work. His supervisor described one time when he was expected to do an oil change, a job that should take 20 minutes. "Joachim would work for 10 minutes or so, get confused and have to start over again. It took him almost two hours to do a simple oil change, and he wasted so many supplies! Eventually, I had to let him go."

Why this works:
  • The change in performance over time is clearly described
  • Symptoms explicitly shown interfering with cognitive abilities
  • Direct quotes from the applicant AND a collateral source (supervisor) strengthen the record
Adapt or Manage Oneself

Can the applicant handle changes in routine, manage psychologically based symptoms, and maintain acceptable behavior in a work setting?

Ineffective Description

Susan doesn't like change. She likes her routine and can get upset with others when things change. She says that when people change things it makes her upset and other people have a problem with that.

Why this is ineffective:
  • No clear link between symptoms and impairment
  • Limited information about impact on ability to maintain employment
Effective Description

Susan experiences significant difficulties adjusting to changes in her schedule or routine. When asked to temporarily change job duties at work from cashier to stocker, she was sent home after yelling at her boss. She explains, "He couldn't just do that. He has no right to change things on me… He said I was yelling, but he was the one in the wrong!" When this writer needed to re-schedule an appointment with Susan over the phone, she became highly agitated and yelled, "It's this stuff that ruins my life!" before hanging up.

Why this works:
  • Quotes and employer details directly illustrate how symptoms caused her to be sent home
  • Inability to adapt to task changes explicitly linked to work difficulties
  • Case manager's own observations provide a concrete example of difficulty managing change
